As a Population Health Coordinator, you will work to make sure that everyone in the community has the information and support they need to stay healthy. You will help people understand how to take care of their health and help them find the right resources. Job Summary
The Population Health Coordinator will help plan and run health programs that help people in Harris County live healthier lives. You will work with doctors, nurses, schools, and other groups to make sure that people in your community know about important health services, like check-ups, vaccines, and health education. You will also help gather information about people’s health so that we can find ways to improve it.
Your main job is to help people get the care they need and make sure our health programs are working well. You will also talk to people in the community, answer their questions about health, and help them connect with resources that can help them.
Responsibilities
As a Population Health Coordinator, here are some of the things you will do:
1. Help Teach People About Health
- Organize fun and informative workshops about health topics like healthy eating, exercise, and how to prevent sickness.
- Make and share easy-to-understand materials that help people learn about staying healthy.
- Work with schools, churches, and other groups to get the word out about health programs.
2. Collect Information and Track Health Data
- Help gather information about the health of people in your community to find out what problems they face.
- Look at the data to see if there are health problems we need to focus on, like high rates of smoking or not enough people getting vaccines.
- Share your findings in reports to help us plan better health programs.
3. Help Organize Health Programs
- Help organize and run health programs that aim to make people healthier and give them access to healthcare.
- Work with doctors, nurses, and other groups to make sure programs are working well and reaching the right people.
4. Help People Find Health Services
- Be a friendly face people can go to for help with their health questions.
- Help connect people to healthcare services they need, such as doctors, clinics, and hospitals.
- Talk to people about things they can do to stay healthy, like getting their annual check-up, taking their medicine, or getting a flu shot.
5. Work with Other Groups
- Team up with other people in the community, like healthcare workers, schools, and social service groups, to work together and make sure people are getting the help they need.
- Attend meetings and work together with other professionals to create better health programs.
6. Keep Records and Write Reports
- Keep track of the work you’re doing and make sure everything is running smoothly.
- Write reports about the health programs to share with your team and the people in charge to show how well things are going and if there’s anything that needs to be fixed.
Qualifications
What Skills You Need:
To do well in this job, here are the things you should be good at:
- Communication Skills: You need to be able to talk clearly and explain things in a way that everyone can understand. You’ll also need to listen to what people say and help them with their questions.
- Organizational Skills: You’ll have to juggle a lot of tasks, so it’s important to stay organized and keep track of everything.
- Problem-Solving: Sometimes health problems can be tricky. You’ll need to be creative and think of ways to solve them.
- Cultural Awareness: Harris County has a lot of different kinds of people. You should be respectful of people’s different backgrounds and make sure your health programs work for everyone.
- Teamwork: Even though you will do a lot of work on your own, you will also work with other people to reach common goals. Being a good team player is important.
- Basic Computer Skills: You’ll need to know how to use simple computer programs like Microsoft Word, Excel, and PowerPoint.
What’s Helpful, But Not Required:
While not required, these skills will help you succeed even more:
- Experience with Public Health Programs: If you’ve worked with health programs before, that’s great! But if you haven’t, don’t worry — we can teach you.
- Knowing About Local Health Resources: If you already know about healthcare services in Harris County, that will make your job easier.
- Speaking Spanish: If you can speak Spanish, that will help you talk to more people in the community.
- Experience with Data: If you have experience looking at and understanding health data, that will be helpful too.
Education and Experience
- Education: You should at least have a high school diploma or a GED. If you have some college education or a degree in public health, nursing, or a similar field, that’s even better.
- Experience: It’s helpful to have at least 1-2 years of experience in a healthcare or community health role. If you don’t have experience but are eager to learn and help others, we still encourage you to apply!
